ABSTRACT

Accessory breast tissue occurs due to failure of resolution of embryonic mammary ridges present from axilla to groin bilaterally. It may be functional or non-functional. This condition is more commonly seen in the females and very few cases have been reported in males. Development of ectopic breast tissue is dependent on hormonal stimulation similar to normal breast.It remains asymptomatic usually and increases in size after a hormonal stimulation at the time or after puberty. Majority of the occurrences are completely benign, but this condition also has the potential to convert into a malignant lesion. Apocrine hidrocystoma is regarded as cystic retention and adenomatous proliferation of apocrine sweat glands. It commonly occurs on the head & neck, but can infrequently be present in other locations like axilla, vulva, penis. This is the report of case of apocrine hidrocystoma with accessory breast tissue in a 30 year old male presented with left axillary swelling. Cytomorphological assessment revealed benign epithelial lesion and a diagnosis was established only after a histopathological examination. It emphasises the role of histopathology in diagnosis of axillary breast lesions

ABSTRACT

Introducción: El fibroadenoma es una lesión bastante común en mama, pero su localización en la región vulvar es extremadamente rara, con alrededor de sesenta casos, descritos en la literatura. Se presenta como una masa unilateral, subcutánea y asintomática, de crecimiento insidioso. El tamaño de este tipo de lesión varía entre 1 y 12 cm en los casos reportados hasta la fecha. A pesar de ser infrecuentes, las tumoraciones a nivel vulvar continúan siendo importante motivo de consulta. La histogénesis de este tipo de lesiones ha sido tema de discusión en los últimos años, sobresalen dos teorías: la presencia de tejido mamario ectópico, y la degeneración tumoral de un grupo de glándulas similares a tejido mamario, descritas como componente habitual de la región. Objetivo: Aportar un diagnóstico diferencial polémico de tumores vulvares. Presentación del caso: Se presenta una paciente de 22 años con diagnóstico histopatológico de fibroadenoma de vulva. Se realiza exéresis de la lesión, la cual al examen macroscópico midió aproximadamente 15 cm en su eje longitudinal (la más grande reportada en la literatura). Conclusiones: A pesar de su baja frecuencia de presentación, el fibroadenoma de vulva es una entidad a tener en cuenta como diagnóstico diferencial de los tumores subcutáneos de localización vulvar(AU)


Introduction: Fibroadenoma is a fairly common lesion in the breast tissue, but its location in the vulvar region is extremely rare, with about 60 specific cases in the literature. It is presented as a unilateral, subcutaneous and asymptomatic mass, with insidious growth. The size of this type of lesion is between 1 and 12 cm in the cases reported to date. Despite being infrequent, vulvar tumors continue to be important reasons for medical consultation. The histogenesis of this type of lesions has been the subject of discussion in recent years; two theories stand out: the presence of ectopic breast tissue, and tumor degeneration of a group of glands similar to breast tissue described as a common component of the region. Objective: To providea polemic differencial diagnosis of vulvar tumors. Case presentation: We present a 22-year-old patient with histopathological diagnosis of vulvar fibroadenoma. Excision of the lesion was performed, which at the macroscopic examination measured approximately 15 cm in its longitudinal axis (the largest reported in the literature). Conclusions: Despite its low frequency of presentation, vulvar fibroadenoma needs to be considered as differencial diagnosis of vulvar tumors(AU)

Resumen Introducción El tejido mamario ectópico es consecuencia de la involución incompleta de la cresta mamaria. Su localización más frecuente es axilar siendo más común en el sexo femenino. Nuestro objetivo fue presentar el caso de una patología infrecuente en su localización. Materiales y Método Se presenta el caso de una paciente de 39 años, con diagnóstico de cáncer de mama ectópica en la región axilar izquierda. Resultados Presentamos el estudio y la resolución quirúrgica de una paciente con cáncer de mama ectópica. Discusión La presencia de tejido mamario ectópico ocurre en el 2%-6% de la población general. Este tejido sufre cambios fisiopatológicos similares a los de la mama normal, entre los cuales se encuentra la malignización. El carcinoma primario de este tejido es infrecuente y su manifestación más común es el tumor palpable. Conclusiones La incidencia de carcinoma en tejido ectópico es de 0,3% de todos los cánceres de mama. El tratamiento debe seguir las mismas recomendaciones que el cáncer de mama normotópico, con igual estadio TNM.


Introduction Ectopic mammary tissue is consecuence of the incomplete involution of the mammary crest. The most frequent location is the axillary region and more common in women. Our gol was present the case of an infrequent pathology in its location. Materials and Method A case of a 39-year-old patient with a diagnosis of ectopic breast cancer in the left axillary region. Results We present the study and surgical resolution of a patient with ectopic breast cancer. Discussion Ectopic breast tissue occurs in 2-6% of the general population. This tissue undergoes pathophysiological changes similar to those of the normal breast, among which malignancy is found. The primary carcinoma of this tissue is infrequent and its most common manifestation is the palpable tumor. Conclussion The incidence of carcinoma in ectopic tissue is approximately 0.3% of all breast cancers. Treatment should follow the same recommendations as those for breast tumors with the same TNM stage.

Vulval fibroadenoma is an uncommon benign tumourthat has been suggested to originate from ectopic breasttissue or mammary like anogenital glands. We report acase of a 26 years old woman who presented with alarge, slow growing painless mass in vulvar region since2 months with a clinical diagnosis of sebaceous cyst/Inclusion epidermoid cyst. Excision was done and onhistopathological examination it was reported as ectopicbreast tissue with fibroadenoma and lactational change.

El tejido mamario ectópico se desarrolla debido a la involución incompleta de la cresta mamaria, de localización más frecuente en la axila y más común en mujeres. Este tejido sufre cambios fisiopatológicos similares a los de la mama normal, entre los cuales se encuentra la malignización. El carcinoma primario de este tejido es infrecuente, y su manifestación más común es la masa palpable. Al igual que en la mama normal, la histología más frecuente del cáncer de mama ectópica es el Carcinoma Ductal Infiltrante y, por su localización atípica, suele diagnosticarse tardíamente, y tendría un curso más agresivo y de peor pronóstico, dado por la mayor cercanía a ganglios, piel y pared torácica. Debido a los pocos datos publicados, el diagnóstico y tratamiento no están bien establecidos, pero los esquemas disponibles son similares a los utilizados en el cáncer de mama normal. Se presenta el caso de una paciente de 41 años, con diagnóstico de cáncer de mama ectópica en la región axilar derecha, sin invasión a distancia, tratada con cirugía, quimioterapia y radioterapia, que evoluciona favorablemente, sin metástasis ni recidivas durante seguimiento.


Ectopic mammary tissue develops due to the incomplete involution of the mammary crest, which is more frequently located in the axilla and more common in women. This tissue undergoes pathophysiological changes similar to those of the normal breast, among which malignancy is found. The primary carcinoma of this tissue is infrequent and its most common manifestation is the palpable mass. As in the normal breast, the most frequent histology of ectopic breast cancer is the Infiltrating Ductal Carcinoma and, because of its atypical location, it is usually diagnosed late and it would have a more aggressive course and a worse prognosis, due to the greater proximity to lymph nodes, skin and chest wall. Because of the few published data, the diagnosis and treatment are not well established, but the available schemes are similar to those used in normal breast cancer. We present the case of a 41yearsold female patient with a diagnosis of ectopic breast cancer in the right axillary region, without distant invasion, treated with surgery, chemotherapy and radiotherapy, whoevolves favorably, without metastasis or relapses during follow-up.

ABSTRACT

Ectopic breast tissue and male breast cancer are both very rare diseases with only a few reports in the literature. Here, we present the first case of ectopic male breast cancer in the perineum. The patient was a 70-year-old man with a palpable mass in the perineum. A wide local excision and inguinal lymph node dissection revealed invasive breast carcinoma of no special type involving the skin and subcutis, and inguinal lymph node metastases. Immunohistochemical staining showed that the tumor cells were strongly positive for estrogen and progesterone receptors and negative for human epidermal growth factor receptor 2. Moreover, no p53 overexpression was observed. Herein, the clinical and pathologic features, as well as a review of ectopic male breast cancer are discussed.

ABSTRACT

The present lesion was the first reported case of a benign intracystic phyllodes tumor in the inguinal region. We report the case of a 51-year-old female patient who presented with an inguinal mass. A clinical diagnosis of malignant lymphoma was considered in this case. The resected tumor was well-circumscribed and showed numerous papillary nodular protrusions into a central cystic cavity (3.5x2.5 cm). The microscopic findings showed hyperplastic epithelium-lined cysts with leaf-like intraluminal epithelium-lined bland stromal projections. The epithelial cell linings were strongly positive for estrogen and progesterone receptors.

ABSTRACT

Nos últimos três anos foram realizadas 30.000 mamografias no Departamento de Diagnóstico por Imagem da Universidade Federal de São Paulo/Escola Paulista de Medicina, sendo que em 0,23% delas foi observado tecido mamário acessório. Relatamos um caso de paciente de 44 anos de idade, feminina, que apresentava nódulo endurecido, aderido ao subcutâneo, em topografia inframamária à direita. À ultra-sonografia observou-se imagem nodular sólida hipoecogênica, com contornos irregulares, medindo 1,4 cm. À ressonância magnética visualizou-se curva de realce tipo II (platô). Após biópsia, foi diagnosticado denocarcinoma mamário. O carcinoma nas mamas acessórias costuma apresentar aspecto histopatológico indiferenciado e disseminação mais precoce, determinando pior prognóstico. Por este motivo, é de fundamental importância sua detecção nos estágios iniciais, permitindo, dessa forma, a instituição precoce do tratamento e possibilitando maiores chances de cura.


Over the last three years 30,000 mammograms were performed in the Department of Diagnostic Imaging of Universidade Federal de São Paulo/Escola Paulista de Medicina. Accessory breast tissue was observed in 0.23% of the individuals. We report a case of a 44-year-old woman presenting a hard nodule adhered to the subcutaneous tissue in right inframammary topography. At ultrasonography, a hypoechogenic solid nodule with irregular contour measuring 1.4 cm was observed. Magnetic resonance imaging showed type II (plateau) dynamic curve. Breast adenocarcinoma was subsequently diagnosed by biopsy. Accessory breast carcinomas generally present with an ill-defined histopathological aspectand early dissemination. Early detection is essential to begin treatment in the initial stages, with better chances of cure.

ABSTRACT

Fibroadenoma is a rare lesion of the vulva. It has been proposed that the origin of the tissue is either ectopic breast tissue or vulvar mammary-like glands (MLG). We report a case of vulvar fibroadenoma in a 33-year-old woman with a right vulvar mass presenting clinically as vulvar cyst. The cut surface of the tumor showed a white and homogenous appearance that was 2.5 cm in maximum dimension. Microscopically, the lesion was well-circumscribed but non-encapsulated. It revealed nodular, glandular and stromal proliferation, consistent with fibroadenoma of breast. Immunohistochemical staining for the estrogen receptor and progesteron receptor revealed nuclear positivity in the epithelium. Smooth muscle actin confirmed the presence of a myoepithelial cell layer. Staining for pancytokeratin showed epithelial cytoplasmic positivity.

ABSTRACT

Ectopic breast tissue may be seen along a diagonal line drawn from axilla to groin and it is rare with only a few reports in the world. There is a relatively frequent occurrence of ectopic breast tissue close to the breast or in the axilla, but the vulvar location is rare. We experienced a case of ectopic breast tissue in the vulva and reported it with brief review of literatures.
